
MIT researchers recently published a finding that should give every business leader pause.
Their 2025 study of enterprise AI deployments, concluded that the core barrier to getting results from AI is not the technology itself, nor regulation, nor a shortage of skilled people. It is learning. Most AI systems, they found, "do not retain feedback, adapt to context, or improve over time."
That sentence deserves to sit for a moment. Billions of dollars invested. Pilots launched across nearly every industry. And the number one reason they fail is that the AI starts from scratch every single time.
If you have tried using AI tools in your business and found the results inconsistent, generic, or frustrating, this is almost certainly why. Not the model. The memory.
What "no memory" actually means
Imagine hiring a brilliant consultant. Talented, fast, articulate. On day one, you spend three hours getting them up to speed: your customers, your processes, your terminology, your standards, the mistakes you made last year that you never want to repeat. The work they produce that day is good.
They come back the next morning with no memory of any of it.
You spend three hours again. The work is good again. Day three: same thing.
This is the default state of most AI tools in most business environments. Every conversation begins with a blank slate. The AI has no knowledge of your organization, your history, your preferences, or the corrections you made yesterday. It will make the same mistakes it made last week. It will ask you to re-explain things you already explained. It will produce output that is technically competent but completely disconnected from your specific context.
This is not a flaw in the technology. It is a feature that most businesses have not yet learned to work around. The ones that figure it out are the ones extracting real value.
Garbage in; confidence out
There is a risk worse than giving your AI no context. It is giving your AI bad context.
When you feed an AI tool outdated documents, contradictory instructions, or information that used to be true but no longer is, the AI does not flag the conflict. It does not say "this seems inconsistent." It incorporates everything it was given and produces output that sounds authoritative. The garbage goes in quietly. It comes out wearing a professional font.
In a small island business environment, the cost of this is not abstract. A customer communication drafted from outdated pricing. A report built on last year's data presented as current. A decision support tool recommending an action that made sense six months ago but not today.
The technical term for this is context poisoning. Here is what it looks like in practice:
During an active production analytics build, an AI assistant wrote status notes into the same document that held stable architectural rules. Those notes were accurate when written. Two weeks and several sessions later, the AI read them as current fact. They weren't. Those unseen errors silently propagated across the entire project. Fourteen documents required cleaning across four sessions to repair the damage. The root cause wasn't a reasoning failure. It was an organizational failure: no system separated information that stays true from information that expires.
That failure had nothing to do with the sophistication of the AI tool. It had everything to do with the discipline of the environment around it.
What governance actually means
AI governance is not a compliance document or a policy your legal team approves and files away. It is the living system of rules, verified information, and correction habits that ensures your AI is working from accurate context every single time it is used.
Two disciplines from outside the technology world are the most practical frameworks for building this system.
The first is CAPA — Corrective and Preventative Action — a quality management methodology with roots in pharmaceutical manufacturing and medical device regulation. Most people in technology have never encountered it. Most people in manufacturing, healthcare, and aerospace know it well. CAPA's core principle is that when something goes wrong, you don't just fix the output and move on. You identify the root cause and implement a prevention that stops the same failure class from recurring. Applied to AI, this means every mistake becomes a documented rule, not a promise to do better.
The second is Knowledge Management — a field of organizational theory most associated with the researcher Ikujiro Nonaka of Hitotsubashi University in Tokyo. Nonaka's central insight is that organizations fail to sustain performance when experienced people carry critical knowledge in their heads without it ever being written down. When those people leave, the knowledge leaves with them. Applied to AI, the implication is direct: the AI cannot learn from experience the way a person does, so the entire burden of organizational context falls on documentation. Either the knowledge is written down and provided, or the AI operates without it.
Together, these frameworks explain both what to build and why. CAPA explains how to improve the system when it fails. Knowledge Management explains how to structure the knowledge the system runs on.
Think of the practical system in three parts.
The first part is standing rules: the short list of things your AI always does and never does, regardless of what it is working on. Not suggestions but guardrails that fire automatically, like a sales rep who has been told in writing to never quote a price without checking the current rate sheet.
The second part is a verified knowledge base: the documents your AI reads before starting any significant work. Not every document you've ever produced. The current, verified, authoritative versions of the information the AI needs to do its job accurately. When something changes in your business, the knowledge base gets updated and the old version gets removed. The AI is always working from what is true now.
The third part is a correction loop: the habit that turns mistakes into permanent improvements. When the AI gets something wrong, you identify why, encode a rule that prevents the same mistake from happening again, and verify that the rule actually works. One mistake is acceptable. The same mistake twice means your governance system failed.
Why this matters more on Guam
The margin for error is smaller here than it is for a Fortune 500 company with a hundred-person technology team and the budget to absorb failed pilots. When an AI rollout goes wrong here, it does not get quietly buried in a quarterly earnings footnote. It affects real operations, real customers, and real decisions made by people who trusted the output.
Guam's Legislature has formed an AI Regulatory Task Force to develop the frameworks that will govern how AI is deployed across the island's workplaces and institutions. The businesses that build strong internal governance practices now will not be scrambling to comply with whatever framework emerges. They will already be operating above it.
The work that actually matters
The AI tools available today are genuinely powerful. The models are not the problem. The problem is the infrastructure of discipline that most organizations have not built around them.
Before you invest in another AI application, ask a simpler question: what system do we have for keeping our AI accurately informed? Who owns that system? How do we correct it when it drifts? How do we prevent the same mistake from happening twice?
The MIT study found that the organizations successfully crossing what they called the GenAI Divide share one thing: they build systems for learning, not just tools for output. CAPA and Knowledge Management are two proven frameworks for building that learning system. They do not require a large budget or a technical team. They require treating AI governance the same way you treat any other operational standard: something worth building, worth maintaining, and worth correcting when it fails.
